I understand that hockey training involves inherent risks, including injury, loss, damage, or accident. I release and hold harmless the organization, coaches, staff, and affiliates from claims arising from participation in this program, except where prohibited by law. I confirm the participant is physically able to participate and I will disclose any medical conditions, limitations, allergies, or concerns that may affect participation. I also grant permission for photos and videos taken during the program to be used for training, teaching, and promotional purposes.
